Reporting to the Deputy Medical Officer of Health, the Associate Medical Officer of Health's duties and responsibilities include: leading, planning, developing and evaluating Public Health programs and services; advising the Medical Officer of Health in matters affecting the operations of Ottawa Public Health; assessing community health problems; improving public relations and interpreting Public Health policies, objectives and roles to the public; monitoring communicable and chronic disease trends and supervising related health promotion, and disease and injury prevention and control programs relevant to the AMOH’s specific areas of focus. EDUCATION AND EXPERIENCE

Fellow of the Royal College of Physicians of Canada (FRCPC) in Public Health and Preventive Medicine

Minimum of 5 years of progressively responsible public health and preventative medicine experience in the public health field with emphasis on program planning, assessing and leading community health problems, and providing medical direction to public health professionals (including experience as a physician within the public health sector)

CERTIFICATIONS AND LICENCES

Physician licensed with the College of Physicians and Surgeons of Ontario

Specifically within our Executive, Occupational and Travel Health programs, you will be meeting with clients one-on-one to provide a variety of counseling, vaccinations, medications and general health advice to travelers going abroad. With regard to immunizations, you will be responsible to review each client’s medical history and make recommendations for safe travel.

Within our Family Health program, you will be responsible for the assessment or care needs upon intake, triaging acute concerns, navigating appropriate services, disease prevention and health promotion as well as chronic and complex disease care and management.

The successful candidate will work throughout all our programs, providing direct nursing support and services to clients and the interdisciplinary team as well as help coordinate and manage their care. At TELUS Health, RN’s provide safe, competent and compassionate care across the lifespan and work very closely with the interdisciplinary team to ensure the highest quality of care.

 

What you’ll do

As part of the team you’ll be responsible for the provision of family health nursing care to the population of clients seen in the family practice service and requires a thorough knowledge of primary care nursing within the scope of CARNA. Areas of focus include: assessing care needs on intake, triage and navigation to appropriate services, disease prevention and health promotion, travel medicine, chronic and complex disease care management, collaboration with the physician and the interdisciplinary team.

Responsibilities include:

  • Individual patient assessment and treatment- health assessment, immunizations, procedures, regular patient follow up to promote health and counsel on disease prevention
  • Triaging acute concerns and providing anticipatory guidance as needed
  • Develop, assess, implement and evaluate care plans for all consultations
  • In depth knowledge of current guidelines, practice standards and medical directives
  • Provide quality, safe, compassionate care to all clients
  • Communicate and work effectively with all other members of the interdisciplinary team
  • Exceptional organizational and interpersonal skills

  • Ability to take initiative and demonstrate proactive/ preventative approaches to care

 

 

Qualifications

 

What you bring

  • BN or BScN from a recognized and accredited program
  • Current CARNA license in good standing
  • BLS Provider course up to date
  • Minimum of 3 years nursing experience (private or public sector)

 

Great-to-haves:

  • Experience with chronic disease management and longitudinal care 
  • Experience assessing and providing travel health immunizations and risk mitigation strategies
  • Experience with Occupational Health 
  • Experience with wound care and management, suture/staple removal and dressing changes
  • Familiarity with ISTM and/or ISTM certification 
  • Computer skills are an asset (EMR, Power Point, Excel, Word)

KEY DUTIES & RESPONSIBILITIES

Program Coordination 

  • Acts as the first point-of-contact for environmental and health inquiries and incidents.
  • Provides guidance to team members on environmental and health matters, and escalates inquiries, where appropriate
  • Coordinates the implementation of environmental, health and safety programs along with related processes, procedures and resources
  • Coordinates environmental, health and safety activities including but not limited to attending tenant OSH committee meetings, attending environmental committee meetings and activities, undertaking incident investigation and resolution, completing equipment and systems registration, developing emergency response procedures and business continuity planning activities, completing and delivering training, completing annual management system re-registration, managing data and generating reports, and conducting environmental compliance audits.
  • Researches environmental, health and safety matters; provides findings and recommendations.
  • Maintains and assists with the creation of environmental, health and safety documentation including but not limited to plans, processes, forms, logbooks, and procedures.
  • Create and maintain documentation including but not limited to instruction sheets, checklists, forms, and training materials.
  • Support construction activity teams (Projects and O&M) by creating, distributing, verifying and maintaining documentation such as work instructions, checklists, training materials
  • Conducts regular inspections of facilities to mitigate any Environmental regulatory issues and/or Health and Safety Concerns before an incident or service call is received.
  • Maintains business continuity and emergency management plans and procedures
  • Participates in field inspections, job observations, health, safety and environment audits and other types of reviews for the purpose of monitoring compliance to environmental and health regulations and identifying risks.  Provides findings about risks and suggestions for improvement.

Incident Investigation, Resolution & Reduction

  • Assists in the investigation and resolution of incidents including root-cause analysis and providing probable causes of incidents and recommendations for resolution
  • Coordinates the implementation of corrective and preventative measures
  • Assists in the identification and analysis of trends.  Communicates observations and provides suggestions for incident reduction measures

Audit Compliance, Data Maintenance & Reporting 

  • Maintains all environmental, health and safety data including but not limited to incident data, and regulatory and process compliance data within applicable databases and all other types of information storage systems
  • Assists in the audit of compliance records and all other environmental, health and safety data
  • Prepares and distributes various environmental, health and safety related reports including but not limited to incident reports
  • Experience with database development, programing and maintenance considered an asset
  • Other duties as assigned

KNOWLEDGE & SKILLS

  • Strong background in environmental management system development and administration, with knowledge of environmental regulations and environmental compliance auditing an asset.
  • Certification in one or more of the following – Certificate in Occupational Health & Safety; Certificate in Environmental Management through a Community College; University degree in Industrial Hygiene, Environmental Engineering, Environmental Sciences, Environmental Studies
  • 1 to 3 years of environmental, health and safety work experience

Located within the Inuvik Regional Hospital and reporting directly to the Supervisor Long Term Care the Registered Nurse (RN), Long Term Care (LTC) provides culturally sensitive care to residents who have medical disorders with predictable outcomes in the 25-bed (4 of these beds are for respite care) LTC Unit. During the day shift during the week there is a Team Leader RN on duty. Outside of regular business hours, the RN, Continuing Care will act as a Responsibility Nurse and be accountable for the overall nursing care of all patients within the unit. In addition, during the day shifts there are 3 Personal Support Workers (PSW)s on duty and on nights there is 1 PSW on duty who are functional team members. Every RN, Long Term Care may be required to act as the Responsibility Nurse when working nights or weekends.


The RN, Long Term Care is a member of the nursing team who provides direct care to residents/patients who have common well-defined nursing diagnoses. These residents are long-term convalescent, geriatric, respite or cognitively impaired residents or palliative care patients who live within the unit. Residents within the Unit are visited a minimum of once per week or more frequently as required by their physician. As a result, the RN independently makes day-to-day decisions regarding individual nursing care plans based on delegated medical orders.


The RN, Long Term Care acts as a patient advocate and facilitates communication between the resident, family and other health care professionals. This will result in a holistic approach to planning, organizing, teaching, and relationship development that will best meet the needs of the resident.

Typically, the above qualifications would be attained by:
This level of knowledge is commonly acquired through the successful completion of a Nursing Degree and 2 years recent experience in a Continuing Care setting.

JOB SUMMARY

Resident Care is responsible for the planning, coordinated development, management and implementation of long term care home nursing and personal care activities, programs, services and initiatives to the residents of a municipal long term care facility. 

In collaboration with the multi-disciplinary team, you provide nursing and personal care to residents, with an emphasis on resident safety and engagement. 

EDUCATION AND EXPERIENCE

Completion of 2 year community college Registered Practical Nurse diploma

Minimum of 6 months of related experience, preferably in a geriatric-gerontology or long-term care setting

CERTIFICATIONS AND LICENCES

Registered Practical Nurse (RPN) current with College of Nurses of Ontario

KNOWLEDGE

  • Basic knowledge of applicable legislation and regulations such as:  Regulated Health Professions Act, Health Care Consent Act, Nursing Act, Mental Health Act, Professional Misconduct and Controlled Act, professional standards, Code of Ethics, practice expectations 
  • Infection control guidelines 
  • Medication administration standards 
  • Nursing documentation standards 
  • Principles of verbal and non-verbal communication 
  • Transfer and positioning techniques for persons requiring long term care
  • General knowledge of computer software applications including word processing and mail applications (e.g.: Word, Outlook)

To autonomously diagnose, order and interpret diagnostic tests, prescribe pharmaceuticals and perform procedures for residents in the Long-Term Care Homes and Services Division within their legislated scope of practice.

 

Major Responsibilities:

  • Delivers primary health care to residents in the Long-Term Care Home in the management of acute and chronic medical conditions, therapeutic management, health promotion and disease/injury prevention in order to deliver comprehensive health services.
  • Conducts focused health assessment using and adapting assessment tools and techniques based on resident needs.
  • Completes health history into resident's situation, including physical, psychosocial, emotional, cultural and ethnic dimensions of health.
  • Performs physical examination and identifies and interprets normal and abnormal findings.
  • Orders diagnostic investigations, and interprets results using evidence-based clinical reasoning.
  • Synthesizes health information using critical inquiry and clinical reasoning to formulate a diagnosis, health risks and states of health/illness.
  • Communicates with residents about the health assessment findings and/or diagnosis, including outcomes and prognosis through the application of knowledge of pathophysiology, psychopathology, epidemiology, infectious diseases, behavioural sciences and family processes when making diagnoses and providing overall therapeutic management.
  • Selects appropriate interventions from a range of non-pharmacological and pharmacological interventions to restore or maintain resident functional, physiological and mental stability to achieve optimal health.
  • Promotes safe resident care by mitigating harm and addressing immediate risks for residents and others affected by adverse events and near misses.
  • Participates with the Medical Director and other senior management staff in the development and implementation of evaluation processes and the identification of strategies to continually improve clinical outcomes and effectively manage risks.
  • Collaborates with senior management in the Division and other community health services in initiatives for the development, implementation and evaluation of systems to promote continuity of resident care across the continuum.
  • Ensures Accreditation Canada standards are evident in daily practice.
  • Ensures resident files and health care records are properly maintained in compliance with applicable legislation and divisional expectations.
  • Identifies, and collaborates with the Medical Director on, educational programs for the care team.

 

Key Qualifications:

  1. Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (Primary Health Care or Adult) and entitled to practise.
  2. Experience as a Nurse Practitioner in long-term care, acute care, rehabilitation, continuing care or primary care.
  3. Experience in long-term care, with providing empathetic/relational care for the elderly and those living in long-term care, and clinical work experience with frail older adults, critical care, and mental health as a Nurse Practitioner.
  4. Certification in Geriatric Nursing from the Canadian Nursing Association or equivalent experience.

 

Must also have:

  • Ability to incorporate knowledge of diversity, cultural safety and the determinants of health in the assessment, diagnosis and therapeutic management of residents and the evaluation of outcomes
  • Ability to exercise independent judgment and to assess situations and problems efficiently and effectively
  • Leadership skills, working well within an inter-professional team and ability to foster effective working relationships
  • Ability to provide clinical supervision, education and mentoring for nursing students, medical students, and other learners, including residents/families
  • Ability to develop and implement evaluation processes and identify strategies to improve clinical outcomes and manage risks
  • Strong and effective verbal and written communication skills
  • Ability to use computer software such as MS Office applications, email and web applications
  • Ability to build and maintain effective relationships and partnerships with a diverse range of stakeholders as well as work effectively within multidisciplinary teams
  • Ability to support the Toronto Public Service Values to ensure a culture that champions equity, diversity and respectful workplaces
  • Ability to work shifts, evenings, weekends and holidays, as required
  • Knowledge of relevant legislation and statutes, including, but not limited to, the Occupational Health and Safety Act and the Fixing Long-Term Care Act
