About the opportunity:
- Performs initial and regular health assessments to maintain patient quality of life.
- Supports development of individualized care plans that focus on the patient’s functional abilities and support network.

About you:

- A Diploma in Practical Nursing and are currently registered with the Nova Scotia College of Nursing.
